How We Help
Every individual's journey is unique, which is why we provide personalized services designed to promote independence, stability, and meaningful community living. Whether you're seeking Support Coordination through the Division of Developmental Disabilities or Pre-Tenancy and Tenancy Supports through New Jersey's Housing Supports Program, we're here to guide you every step of the way
Support Coordination
For adults (21+) enrolled in New Jersey's Division of Developmental Disabilities (DDD), a Support Coordinator serves as the primary point of contact for planning and coordinating services. Similar to a case manager in other human service settings, a Support Coordinator works alongside individuals and their families to develop a personalized service plan, connect them with appropriate resources, advocate for their needs, and monitor services to help them achieve their goals.
Our role is to help individuals navigate the DDD system with confidence while promoting independence, informed choice, and meaningful community involvement.
